Withdrawal requests will not be processed until all outstanding issues with the institution is resolved. Student must get clearance signatures from the following offices to certify that the student has paid all financial obligations or fulfilled responsibilities for service being provided. International student with F-1 visas must consult with the PDSO, DSO before submitting this form.
